AVAILABILITY
PCCI is always available for veterans in need of housing assistance. For the month of July, 2025, SSVF assessment appointments for veterans in need of Homelessness Prevention (HP) assistance are limited as veterans who are staying on the streets must take priority. Our staff can still assist veterans in need of HP assistance through consultation and referrals.
If you are at risk of losing housing, read the “Eligibility” box to the right before completing the form on this page.
If you are temporarily staying with friends or family, couch-surfing, paying for a hotel, are in a recovery program not paid for by the VA, or are staying in an institution where you have been for 90 days, complete the form and an Intake Coordinator will contact you shortly to complete an SSVF eligibility screening.
